5 million deaths a year later, Spain’s Tagus River just tested positive for resistant bacteria

Scientists monitoring 19 sites along Spain's Tagus River over nearly a year found antibiotic-resistant bacteria and faecal contamination linked to sewage and farm runoff.

Antimicrobial resistance already kills close to 5 million people a year worldwide, according to a widely cited Lancet study, and Spain’s Tagus River has just offered a fresh reminder of where that resistance can quietly take root. A year-long study by researchers at the University of Castilla-La Mancha has detected antibiotic-resistant bacteria and signs of faecal contamination at multiple points along the country’s longest river.

The team monitored 19 stations spread across the Tagus over nearly twelve months, publishing the results in the journal Environmental Research. Rather than finding one polluted hotspot, they found the contamination spread out across the length of the river, varying in intensity from site to site but present almost everywhere they tested.

The likely culprits are familiar ones: human sewage, urban runoff and agricultural waste, including manure from livestock treated with antibiotics. Once those inputs reach the river, the study suggests, they create conditions where resistant bacteria can survive and exchange resistance genes with other microbes, a process that is much harder to track and control outside a hospital setting.

The 2019 global toll cited by the Lancet, nearly 5 million deaths associated with bacterial antimicrobial resistance and 1.27 million directly attributable to resistant infections, is the backdrop against which researchers are reading these river results. Separate work published in Nature Reviews Microbiology has pointed to rivers and wastewater systems specifically as reservoirs that let resistance circulate between people, animals and the environment.

Even well-run treatment plants do not catch every resistant organism before discharging water back into the river, and the study’s authors are careful to note that finding these bacteria in the water does not mean people risk infection through casual contact with the river. The concern is more about what it signals: that resistance genes are already circulating in the wider environment, not just inside clinics.

Toledo’s stretch of the Tagus has always carried significance for Spain, ecologically and economically, and the researchers behind the study are now pushing for stronger monitoring programmes, upgraded wastewater treatment, and more disciplined antibiotic use in both medicine and farming, arguing that keeping rivers clean may prove just as important to the resistance fight as developing new drugs.
